Cancellation Policy
No-shows, cancellations and reschedules are costly to the practice and prevent others patients from utilizing that appointment time. Failure to provide a notice of more than 2-3 business days will result in a charge. This charge will be your responsibility and billed directly to you. Please help us to avoid this charge and serve you better by keeping your scheduled appointment and or procedure. You will not be able to schedule another appointment/procedure until the charge has been paid.
The following fees will be incurred if the appointment is cancelled with a notice of less than 2 business days:
- $50 – Office appointments
- $50 – Office procedures / office testing
- $25 – Infusion appointments
The following fees will be incurred if the appointment is cancelled with a notice of less than 3 business days:
- $100 – Iowa Endoscopy Center/ Ankeny Surgery Center procedures
- $100 – Outpatient hospital procedures

Closed on Memorial Day, Christmas Day, and New Years Day.
